Well-rounded health and safety investigations

When you need to understand what happened and why, sticking to a solid process is important so that all issues are investigated. At First 4 Safety, we follow the ICAM process to investigate accidents and incidents in the workplace. By gathering, organising and analysing investigative data, we’ll help you identify the root cause.

We are thoroughly trained to lead investigations and hold the following ICAM certificates:

  • Investigate Incidents (PMAOHS310B)
  • Participate in the investigation of incidents (BSBOHS508B)

  • Conduct safety and health investigations (RIIOHS301A)

  • Produce an occupational health and safety incident investigation (US17601)

Although process is important, we also know from experience that handling this work with sensitivity is essential. So as well as investigating thoroughly, we are also here to help you learn from the findings, make any necessary changes and obtain a commitment from workers at all levels.

Accidents and incidents can happen in any business.

When they do occur, they need investigating to prevent re-occurrence and keep the rest of your team safe.

We can also provide organisations with an additional safety net by peer reviewing internal investigation procedures. We can focus on a number of areas depending on the client need:

  • Communicating loss after the accident
  • Identifying and explaining root cause and contributing factors

  • Correct use of the ICAM tool

  • Conclusions made by the investigation team

  • Scope of recommendations

  • Development of key learnings

  • Report structure – is it factual, can it stand alone?
